DESCRIPTION OF HOW THE PREMISE OF THE ARTICLE APPLIES TO YOUR SETTING IN THE INTEREST OF IMPROVING PATIENT OUTCOMES.

Conduct a library search to find an article about a research study or evidenced based initiative that was written by an interdisciplinary team composed of at least two different disciplines i.e. FNP, MD, MSW etc. with the purpose to improve patient outcomes in the primary care setting.

Plan an informal presentation for the interdisciplinary team in your student clinical practice setting. Present an overview to colleagues of the initiative described in the article and discuss ways it may be applicable to the clinical area in the interest of optimal patient outcomes.

Write a 3 – 4 page paper exclusive of references, which include the following:
