Pippa has lived there ever since her husband, Alan, died two years ago. Pippa has Parkinson’s Disease that has been controlled with a combination of Levodopa and Entacapone. Recently however, her tremors have become increasingly unpredictable and variable. Pippa’s dyskinesia has become noticeably worse; Pippa’s doctor has prescribed Amantadine for this. Susan is married to Tom Carter and they have two children; a daughter, Jenny, aged 15, and a son, Deon, aged 17. Susan and Tom both work in the city; about 10 km from home, often getting home late from work. Jenny and Deon usually arrive home about 4.00 – 5.00 PM from school and after school activities. They check on their grandmother most afternoons. Susan thinks that Pippa may have fallen a few times recently, however Pippa does not admit to this. Pippa has some unexplained bruising on her arms and legs. Susan stated she thinks Pippa may be in pain. In addition, Pippa seems to be short of breath and has been sweating a lot. The family has noticed that Pippa has appeared confused at times and sometimes mistakes Deon for her dead husband, Alan. On occasion Pippa has forgotten to eat or to take her medications. Pippa is very independently minded and refuses assistance from either Susan or Tom to help her with her activities of daily living. Pippa says “they are doing enough by having her live with them in her cubbyhouse”. Susan has contacted the community health centre to identify what assistance is available to help both Pippa and Susan’s family to maintain Pippa’s independence at home. They do not want to place her into care and prefer to care for her with whatever help is available, so as to maintain her dignity and independence.
